Call our office today to identify what services you need, i.e. outpatient therapy, medication management, or both. A screening process is conducted to identify if outpatient services are for you. If patient needs are complex, we may refer you to other agencies. 

This can be done one of two ways. The first way is by completing a "Release of Information" form from the previous provider. The records request will then be sent to Phoenix Rising.   The second is by completing the "Release of Information " form at Phoenix Rising. We would then send the request to your previous provider.
